To register for the YogaWorks 200 Hour Yoga Teacher Training program:

- Print and fill out the form
- Submit your deposit payment of $150.00 online or bring/mail it to the Semperviva Lifestyle Store at 2608 West Broadway along with your application form
- Your application will be reviewed immediately
- You will be notified of approval within 3-5 business days
